3. Two (2) copies of PSA/NSO birth certificate (1 original, 1 photocopy)
4. Two (2) copies of baptismal certificate (1 original, 1 photocopy)
5. Two (2) copies of marriage contract (1 original, 1 photocopy)
6. Commitment letter from Catholic parents not married in church
7. Signed agreement form for non-Catholics
8. For parents who are single (single-unmarried, single-separated, or widowed): letter stating who is/are authorized to transact with the school on their behalf
9. Non-refundable application and testing fee of Php500
10. For foreign students: passport, study permit and alien certificate (to be issued by the Bureau of Immigration and the Department of Foreign Affairs)
ADMISSIONS PROCEDURE 1. Parent or guardian submits the requirements to the Admissions Office.
2. Applicant takes the Admissions Test on the scheduled date.
3. Parents and applicant come for an interview.
4. Parent gets test results personally from the Admissions Office five (5) working days after the test. No inquiry regarding test results will be entertained by phone.
5. Parents attend the Parent Partnership Program.
Elementary
Grade 1 • able to read short stories in Filipino and English
• able to answer orally and in written form questions about the story
• presently enrolled in a Preschool (Kinder level or its equivalent)
• has Learner’s Reference Number (LRN)
Grades 2 to 6 • has satisfactory grades in all subjects (no grade lower than 83)
• has satisfactory conduct
